Do Medicare supplement premiums increase with income?

No, Medicare Supplement (Medigap) premiums do not increase with your income. Medigap plans are sold by private insurance companies, and your premium depends on factors like your age, location, the plan letter you choose, and sometimes tobacco use. Income never enters the equation. Here's where income does matter. Your Part B premium and Part D drug coverage can rise based on your income through something called IRMAA, the Income-Related Monthly Adjustment Amount. Social Security uses your tax return from two years prior to decide. The standard 2026 Part B premium is $202.90 a month, but higher earners pay more on top of that.
