Aflac Medicare Supplement Review: Tier One Insurance Company and What Aflac Charges
By Anthony Orner, Independent Licensed Medicare Insurance Broker · Filed-rate data as of October 17, 2026 · Source: CSG Actuarial
Quoted “Tier One Insurance Company” or “American Family Life Assurance Co of Columbus”?
Both are Aflac. New Jersey's state list names Tier One Insurance Company as the underwriter of Aflac's Medicare Supplement. American Family Life Assurance Company of Columbus is the legal name of Aflac's main insurance company, and the name CSG Actuarial's rate data files these rates under.
Aflac files Plan G at $151.64 to $255.10 a month at 65 in the 23 states below. Its best Plan G spot is Kansas, 6 of 25. It sits in the cheaper half of the board in 13 of the 23. It is not the cheapest Plan G in any of them.
Plan N tells a better story: it ranks higher than Aflac's Plan G in 20 of 23 states. The filing history is less kind: 21 states took double-digit Plan G increases in both 2025 and 2026.
Carriers pay the broker; the price you pay is the same either way.

Plan G and Plan N benefits are standardized by federal law. An Aflac Plan G pays the same claims as any other company's Plan G; only the premium and its increases differ.
Three Names, One Policy
Aflac is the brand name you know. Tier One Insurance Company is the Aflac company New Jersey's state list names as underwriter of the Medigap policy; check the company named on your own application. American Family Life Assurance Company of Columbus is the name Aflac's main insurer has used since 1964, and the one CSG files these rates under.
Aflac itself is old: it was founded in 1955 in Columbus, Georgia. Its Medigap line is far younger, dated 2022 in CSG's file, so there are only a few years of filings to judge it on.
Two Straight Years of Double-Digit Filings
In every state on this page except New Hampshire and Delaware, Aflac filed Plan G increases of 10% or more in 2025 and again in 2026. The 2026 filings run from 9.9% to 25%, with a median of 18%. The steepest, 25%, landed in Arizona and Montana.
The earlier record was milder: 19 of these states took a 2024 filing under 10%. That shift is why a new-business rank is only half the picture. If you already own Aflac and got a renewal letter, ask what a switch would cost at your age now. Outside a guaranteed-issue right, most states let the new carrier ask health questions.
Where Aflac Competes: Plan N
Aflac's Plan N sits in the cheapest third of the board in 13 of 23 states. Its strongest showings are West Virginia ($114.44, 5 of 25) and New Jersey ($129.82, 4 of 20). Plan N costs less because you pay up to $20 at some office visits and up to $50 at the ER, plus any Part B excess charges.
