$239 a month separates the cheapest filed Plan G in North Carolina from the most expensive one, for a policy that pays identical claims. Over 20 years that gap alone is roughly $57,360, before a single rate increase.

Plan G Premium Spread by Age

Plan G pays everything Original Medicare leaves behind except the Part B deductible ($283 in 2026). Since 2020 it is the fullest plan a new enrollee can buy. Each row is the cheapest and the most expensive filed carrier at that age; the spread column is what picking the wrong one costs per month.

Plan N Premium Spread by Age

Plan N is the same policy with two cost-sharing edges: a copay of up to $20 for an office visit and up to $50 for an emergency room visit, and no coverage for Part B excess charges. In North Carolina the cheapest filed Plan N at 65 is $78.54, which is $23.30 a month under the cheapest Plan G, about $280 a year. Four office visits a year is $80 in copays against that.

Why the Gap Is This Wide in North Carolina

Two things. The first is rating method. North Carolina lets a carrier price by attained age, issue age, or community rate, and the three methods produce different curves from the same starting point. You can see it in the table: the highest filed Plan G climbs from $315.20 at 65 to $341.00 at 69, while the cheapest filed carrier does not move between 65 and 67.

The second is the size of the field. With 29 carriers filing, a few are new blocks priced low to win business, and a few are older blocks priced to the claims of the people already in them. The carrier that is cheapest for a 65-year-old today is often not the carrier that stays cheapest, which is why the filing history matters as much as the price.

Rank moves when filings land. Mutual of Omaha's North Carolina block filed +25.01% effective April 2025 and +20% effective May 2026; a carrier that is fourth-cheapest today can be tenth after one filing. Ask for the filing history before you take the lowest number.

The Household Discount, and Why Couples Should Be Quoted Twice

6 of the North Carolina Plan G carriers file a household discount of 11.1% to 20%. The rules are carrier-specific. Some want a spouse on the same carrier. Some accept any adult in the home who holds a Medigap policy from anyone.

For a couple, the discount can move a carrier from third-cheapest to first, so we quote every carrier with it and without it. A single carrier's website quotes you once.

Tobacco Rating: the North Carolina Difference

North Carolina lets carriers rate for tobacco, and in this snapshot every carrier that quoted did: 38 of 38 applied a surcharge, from 10% to 221%, averaging about 31.6%. (New Jersey blocks the surcharge during the six-month open enrollment window. North Carolina has no such rule.)

If you use tobacco, throw out the non-tobacco ranking. The cheapest carrier for a non-smoker and the cheapest for a smoker are usually different companies, and the gap between two carriers' surcharges alone can add up to hundreds of dollars a year.
