How to Apply for LifeShield Medicare Supplement

By Anthony Orner, Independent Licensed Medicare Insurance Broker · NPN 20586791 · Filed rates pulled September 9, 2026 to November 5, 2026 · Source: CSG Actuarial

LifeShield Medicare Supplement takes no applications on its own website. A licensed agent contracted with LifeShield submits it. We're an independent, licensed agency contracted with LifeShield. We're not LifeShield, but we can help you apply in 20 states.

Its Plan G runs $125.11 to $243.14 a month for a 65-year-old woman across the 23 states on our rate boards, and ranks in the top three in 13 of them.

The carrier pays the broker. Your premium is the state-filed rate either way.

What the Form on LifeShield's Website Does

LifeShield's site ends in a short form (checked November 4, 2026). Name, contact details, and a box to tick for Medicare Supplement. Ticking it authorizes “a representative of LifeShield National Insurance Co.” to make contact by email or phone.

That is a contact request. It doesn't price a plan for a ZIP, and nothing in it is an application. LifeShield sells its Medicare Supplement through licensed agents contracted with the company, and the agent files the application.

Medicare.gov stops short too. Its Medigap tool lists LifeShield beside a price range and a phone number, and it can't enroll anyone in a Medigap policy. Medigap is bought from the insurance company. For LifeShield, that means through an agent.

CSG Actuarial licensed filings, pulled September 9, 2026 to November 5, 2026. Female, 65, non-tobacco, individual rate, no household discount, one sample ZIP in each flagship county. Your own quote moves with ZIP, age, sex and tobacco use.

Cheapest or close to it

Tennessee, Wyoming, Arizona, Colorado, Kansas, South Carolina, Indiana, Virginia, Iowa, West Virginia, Alabama, Oklahoma and New Jersey. No company filed a lower Plan G in Tennessee. Elsewhere in this group LifeShield runs $6.44 to $32.05 a month over the cheapest filing.

Mid-table

Mississippi, Ohio, Delaware, Arkansas, North Carolina, Nebraska, Georgia, Texas and Florida. Under the state median in 7 of them, but not near the front. Worth a side-by-side before you apply.

Compare before you apply

Pennsylvania: 15 of 27 at $207.13, $38.54 over the cheapest. Plan G benefits are federal, so a cheaper Plan G pays the same claims. Call and we'll price the field.

Often Second to Bankers Fidelity

In 8 of the 12 states where LifeShield is second or third, the cheapest Plan G on the board is Bankers Fidelity or its sister insurer, Atlantic Capital Life. The gap at 65 runs $9.21 to $32.05 a month on our boards.

Neither company takes applications on its own website, and one call prices both. How the Bankers Fidelity application works.

Before You Apply: What Moves the Premium Later

LifeShield prices on attained age in 19 of its 23 states, so the premium steps up with each birthday. Arizona, Florida and Georgia are issue-age: the price follows the age you buy at, and only rate filings move it. Arkansas is community-rated, one price at every age.

Filings move every policy in a state at once. LifeShield has a 2026 filing in 22 states, from +9.0% in 7 states to +23.2% in Nebraska.

The full filing record by state is in our LifeShield National review.

Finding a LifeShield Agent

Where we can take your application

Alabama, Arizona, Arkansas, Colorado, Delaware, Florida, Georgia, Indiana, Kansas, Mississippi, New Jersey, North Carolina, Ohio, Pennsylvania, South Carolina, Tennessee, Texas, Virginia, West Virginia and Wyoming.

Distance doesn't matter. Any agent contracted with LifeShield and licensed in the applicant's state can file it. Our office is in Cherry Hill, NJ.

Where we can't

LifeShield files in Iowa and Nebraska, where we're not licensed yet. Oklahoma is out by choice: we leave some birthday- and anniversary-rule states alone.

Anywhere else, call. We'll say plainly whether we can place it rather than take an application we can't submit.

How the Application Goes

  1. 1

Price first. A ZIP and a date of birth price Plan G and Plan N before anything is signed. LifeShield not in the online list for that ZIP yet? Call and we price it by phone. Where it isn't the better buy, we say so.

  2. 2

Timing sets the questions. In the six months that start with Part B at 65 or older, LifeShield must accept the application and can't charge more for health conditions. Later, expect health questions unless a guaranteed-issue right applies.

  3. 3

Have three things ready. The Medicare number, the Part A and Part B start dates from the card, and the bank account for the premium draft.

  4. 4

Sign, then keep 30 days. Once the policy is issued, a 30-day free look lets you cancel for a full refund.
