Does Braven Health have any Medicare Advantage plan for 2027?

Not an individual one. CMS's 2027 plan crosswalk marks all 10 Braven Health plan IDs sold in New Jersey in 2026 as terminated, with no 2027 plan taking them over, and CMS's 2027 landscape lists no Braven plan for New Jersey. Horizon, Braven's parent, keeps 2 special-needs plans in Medicare Advantage for 2027 and no individual plan. Employer-group Braven plans are not in CMS's public files, so this page does not cover them.

Which Braven plans end on December 31, 2026?

Braven Medicare Choice (H0885-001); Braven Medicare Freedom (H0885-002); Braven Medicare Choice (H0885-003); Braven Medicare Choice (H0885-005); Braven Medicare Choice (H0885-006); Braven Medicare Choice Plus (H0885-007); Braven Medicare Freedom (H0885-008); Braven Medicare Freedom (H0885-009); Braven Medicare Salute - No Rx (H0885-010); Braven Medicare Salute - No Rx (H0885-011). The plan ID is on your member card and on the non-renewal letter. The largest, Braven Medicare Choice H0885-001, had 21,458 members in September 2026 across Essex, Hudson, Monmouth, Ocean, Union.

Can I buy a Medigap plan with no health questions if my Braven plan ends?

Yes. When a Medicare Advantage plan is non-renewed, federal law gives you a guaranteed-issue right to Medigap Plans A, B, D, G, K, L from any insurer that sells them in New Jersey (Plans A, B, C, F, K, L if you were eligible for Medicare before 2020). The insurer cannot turn you down, charge you more for your health, or make you wait for pre-existing conditions. The right runs from 60 days before your coverage ends, November 1, 2026, until 63 days after it ends, March 4, 2027. Keep the letter; it is your proof.

Does Horizon sell Medicare Supplement plans in New Jersey?

Yes. Horizon Blue Cross Blue Shield, Braven's parent, files Plan G at $179.94 a month at 65 in the CSG file. Your guaranteed-issue right is not tied to Horizon or Braven: every one of the 19 carriers in the table has to take you for the guaranteed plans during the window.

Do I need a Part D plan if I move from Braven to Medigap?

Yes, unless your Braven plan had no drug coverage. A Medicare Supplement never covers prescriptions. Pick a stand-alone Part D plan between October 15, 2026 and December 7, 2026 so it starts January 1 with the Medigap plan. The 2026 Part D out-of-pocket cap was $2,100; CMS sets 2027's.

What happens if I do nothing before December 31?

On January 1 you are in Original Medicare with no Medicare Advantage plan and no drug plan. Your guaranteed-issue Medigap right still runs to March 4, 2027, and a special enrollment period for a new Advantage plan runs December 8, 2026 to February 28, 2027. But a Medigap plan that starts in February leaves January uncovered beyond Original Medicare's 2026 Part B deductible of $283 and 20% coinsurance, and a Part D plan picked late brings a late-enrollment penalty if you go 63 days without drug coverage.

19 carriers file Plan G in New Jersey. At 65 the filed rates run $147.88 to $260.98 a month, and at 75 they run $187.90 to $351.00 (CSG Actuarial, non-tobacco, pulled September 11, 2026). New Jersey carriers file one rate for the whole state, so your county does not change the price. 17 of the 19 price by attained age, so the rate follows your age up; AARP / UnitedHealthcare files a community rate that does not.

Braven Health has no individual Medicare Advantage plan in CMS's 2027 list. Its 10 New Jersey plan IDs end December 31, 2026, and they held about 42,000 members in September 2026 across 21 counties. If your letter is a non-renewal notice, federal law lets you buy a Medicare Supplement with no health questions until March 4, 2027.

The right covers Plans A, B, D, G, K, L from any of the 19 carriers filing in New Jersey (Plans A, B, C, F, K, L if you were eligible for Medicare before 2020). CMS publishes two files each fall: the Service Area Reduction file, which names every plan ID not offered next year, and the plan crosswalk, which says whether a retired ID is renewed under a new one. Every Braven ID is in the first file and marked terminated in the second, so no Braven member is being moved into another Braven plan. Match the ID on your card.

Plan G in New Jersey, 19 Carriers, Ages 65 to 80

New Jersey carriers file one rate for the whole state, so a Braven member in Hudson County and one in Ocean County see the same table. Most Braven members are past 65, so the table runs to 80. 17 of the 19 carriers price by attained age, which is why the 75 column sits above the 65 column.

A few carriers file a separate guaranteed-issue rate class that this pull leaves out, so a guaranteed-issue quote can differ from the table. Plan N at 65 starts at $107.91 with 20 carriers.

The Dates

October 15, 2026 to December 7, 2026: the window for a replacement Advantage plan or a Part D plan with a January 1 start. Medigap is not bound by it, but November is the month to apply.
November 1, 2026: the guaranteed-issue Medigap window is open by this date at the latest, 60 days before coverage ends. A letter in hand opens it sooner.
December 31, 2026: every Braven individual plan stops paying claims.
December 8, 2026 to February 28, 2027: the special enrollment period a non-renewal opens for a new Advantage plan.
March 4, 2027: the guaranteed-issue Medigap right closes, 63 days after coverage ended.

Do nothing and on January 1 you are in Original Medicare alone: no drug plan, the 2026 Part B deductible of $283, and 20% of Part B costs with no cap. The Medigap right still runs to March 4, 2027, but a plan that starts February 1 leaves January open.

Apply in November or early December and the Medigap plan starts January 1.

December 7 is the Part D deadline, not the Medigap one. Have the non-renewal letter and your member card when you call; the plan ID on the card tells me which table row you are in.

Two More Rights Worth Checking

If Braven was your first Medicare Advantage plan and you joined it when you first got Medicare at 65, less than 12 months ago, the trial right applies: any Medigap plan sold in New Jersey, from any carrier, no health questions. If you dropped a Medigap policy to try Braven for the first time, less than 12 months ago, you can return to that policy if its carrier still sells it, or take Plans A, B, D, G, K, L. Both run on the same clock as the non-renewal right.

Healthy enough for health questions? Then every plan from every carrier is open, Plan N included, at the standard rate. Apply while the guaranteed-issue right is still open, so a decline on an underwritten application still leaves you the guaranteed plans.

AmeriHealth Ends 2 PPO Plans Too

AmeriHealth Medicare Enhanced (H5361-002) and AmeriHealth Medicare Ultimate (H5361-004) are marked terminated in the same CMS crosswalk, in Atlantic, Burlington, Camden, Gloucester, Mercer, Middlesex, Ocean: 5,414 members in September 2026. AmeriHealth Medicare Core (H5361-001) continues but leaves Atlantic, Burlington, Gloucester, Mercer, Middlesex, Ocean. Those members hold the same guaranteed-issue right on the same dates.

Veterans: Braven Medicare Salute - No Rx (PPO) (H0885-010) and Braven Medicare Salute - No Rx (PPO) (H0885-011) end with the rest.
