Alabama Medicare Costs in 2026: Part B, Plan G and Plan N
By Anthony Orner, Independent Licensed Medicare Insurance Broker · Alabama filed rates pulled September 10, 2026, CSG Actuarial · Jefferson County (ZIP 35006)
Part B plus Plan G costs a 65-year-old in Jefferson County $326.96 to $521.11 a month in 2026, from CSG Actuarial filings pulled September 10, 2026. The $202.90 Part B premium never moves. The Plan G line does: $124.06 at Bankers Fidelity (Atlantic Capital Life), $318.21 at Guarantee Trust Life, 22 carriers between them. Identical benefits under federal law, so the whole $194.15 is price. Twelve months of it is $2,330; twenty years of it, with no increase at all, is $46,596.
Alabama is one of the few states where Plan F still outnumbers Plan G: 52,176 policies to 46,855 at the December 2023 NAIC count. 11 carriers filed increases of 15% or more on Plan G this year, so the number on this page matters less than the filing behind it. Rates are set by rating area, 16 of them for 67 counties, and the same Bankers Fidelity (Atlantic Capital Life) policy is $114.87 in Autauga County.

Part B and the highest filed Plan G, Jefferson County
156% more for a policy that pays the identical claims.
What both cover is set in federal statute: the $1,736 Part A deductible each benefit period, then everything Medicare approves after the $283 Part B deductible, which stays with you on G and on N. The left column of each bill is Medicare's and identical. Only the right column is the carrier's choice.
Halfway down the Jefferson County list sits HealthSpring Insurance (HCSC) at $179.48, a $382.38 bill. Statewide the median Plan G filing is $173.07; 5 of the other 27 states in our file have a lower floor than Alabama's $114.87. Among the flagship counties we track, Jefferson County's $124.06 ranks 8 of 28. Part D is not in these totals because it depends on the drug list.

Plan N in Jefferson County: Smaller Premium, Two Copays
The lowest Plan N here is Bankers Fidelity (Atlantic Capital Life) at $94.21, $29.85 under its own Plan G and $358 a year. For that, Plan N hands back two copays: up to $20 at some office visits, up to $50 at an emergency room that does not admit you. It also leaves Part B excess charges to the policyholder, and Alabama has no law forbidding them; Plan G pays them. 21 carriers file Plan N in Jefferson County, $94.21 to $272.00, with American Benefit Life at the median $125.20. The statewide Plan N floor is $87.23, in Autauga County.

4 Alabama Carriers Discount a Second Policy in the House
Only 4 of the 22 carriers file a household discount in Alabama, and they range from 11.1% to 20%. The biggest belongs to HealthSpring Insurance (HCSC): 20% off a $179.48 Plan G leaves $143.58. Bankers Fidelity (Atlantic Capital Life), the cheapest single-applicant line, files none. Once discounts apply, AARP / UnitedHealthcare is the lowest line at $135.56.
The other 18 carriers give nothing for a spouse. Two adults at one address is the usual test, but every carrier phrases it differently, and some require both to hold a policy with them. We quote a couple two ways, together and apart, because the winning carrier changes.

How 20 of 22 Alabama Carriers Price Age
Attained-age rating covers 20 of the 22 Jefferson County carriers: the premium steps up on birthdays and the block filing lands on top of the step. Blue Cross Blue Shield is the single issue-age line at $237.00; it freezes the age at purchase, so only block filings move it afterward. AARP / UnitedHealthcare is the single community-rated line at $162.54, with a dearer class filed at $565.28; age never enters its price. The highest Plan G filed anywhere in Alabama is $318.21, from Guarantee Trust Life.
19 of the 22 carriers show a 2026 effective date on their Jefferson County rate, the latest being New Era Life on September 1, 2026. Alabama had 169,782 Medigap policies in force at the December 2023 NAIC count: 52,176 on Plan F (31%), 46,855 on Plan G (28%) and 10,743 on Plan N. Plan F cannot be sold to anyone who became eligible for Medicare on or after January 1, 2020, so this page prints no Plan F table.
Methodology
- Every premium is a carrier rate filing pulled from CSG Actuarial on September 10, 2026 and rendered from the Alabama data file. Nothing here was typed by hand.
- Profile: female, non-tobacco, individual, no household discount. Discounted lines are computed from the filed percentage.
- Jefferson County (ZIP 35006) is the flagship county. Each key county is quoted at the ZIP with the largest land overlap; Autauga County represents the rating area that covers 44 counties.
- A gap is the highest filing minus the lowest for the same plan letter, age and county. Twenty-year figures multiply the monthly gap by 240 and assume no increase.
- Carrier counts are the carriers that returned a filed quote for this profile on the pull date. Entries, exits and closed blocks change them.
- Alabama prices in 16 rating areas; the cheapest-carrier line spans $18.38 across them at 65.
- The Part B premium and both deductibles are 2026 CMS figures imported from a single file. Plan G does not pay the Part B deductible.
- Enrollment counts are statewide by plan letter, as of December 2023, from the NAIC Medicare Supplement Experience Exhibit via AHIP May 2025 report. Direct-to-consumer and guaranteed-issue rate classes are excluded from the tables.
- Filing rows come from CSG Actuarial's records for January 2026 to September 2026. A "No" row is a filing whose reported name matched nothing in the quote table and is never paired with a rate.
