Old Surety Life files the cheapest Plan G we track in Nebraska, $136.63 in Douglas County, for a non-smoking woman at 65. In Texas, the identical company ranks 19 of 26, 44% above that state's floor. Unrated by AM Best and carrying a Weiss D safety rating, the Oklahoma City carrier took a rate filing of 15% or more in 6 of its 7 states in 2026, topping out at +30.0% in Montana.

Every filing CSG holds for this carrier prices on an issue-age basis, so a policyholder's premium does not rise on birthdays the way it would with an attained-age carrier. It has not stopped the company from filing large block-wide increases: the record below runs back to 2018, and six of the seven states have already taken one this year.

Issue-Age Pricing, No Plan N, and What That Means

Old Surety Life prices every Plan G filing in our footprint on an issue-age basis: the premium is set by your age when you buy the policy and does not climb again just because you had a birthday, unlike an attained-age carrier whose bill rises every year on top of any filed increase. That is a real structural advantage over some of the carriers on the state tables linked below.

It is not protection from the filings themselves. The 6 rate changes of 15% or more logged above hit the whole block at once, issue-age or not. And Old Surety Life files no Plan N in any of the 7 states we track — every filing on record is Plan G — so a shopper who wants Plan N's lower premium in exchange for its office and ER copays and Part B excess-charge exposure has to compare a different carrier for that letter.

Plan G itself pays everything Original Medicare leaves behind except the $283 Part B deductible, the same as any other company's Plan G; the plan letter is federally standardized and identical no matter who sells it.
