- Kansas was quoted one ZIP per county; counties whose carriers all file identical rates for every plan and age were merged into rating areas, 28 in all, and each key county is quoted inside its area.
- Left out: direct-to-consumer classes, guaranteed-issue classes, bundled Plan G variants, and any row above four times the state median. Carrier identity is the NAIC code.
