A 65-year-old in Charlotte pays $304.74 to $518.10 a month in 2026 for Part B and a Medigap Plan G together, across 29 filed carriers (CSG Actuarial, 2026-07-12). Yearly that is $3,657 against $6,217. The $202.90 Part B premium is fixed. The Plan G half, $101.84 to $315.20, is the half you pick.

Read the table down the columns and the age curve shows up. The cheapest filing moves from $101.84 at 65 to $103.87 at 69. The dearest travels from $315.20 to $341.00 over the same four years, so the gap widens from $213.36 to $237.13. That is what attained-age pricing looks like: 27 of the 30 carriers in the September 10, 2026 county pull raise the premium on each birthday, on top of whatever they file on the block.
Plan N Buys the Premium Down and Sells You Copays
The cheapest filed Plan N at 65 is $78.54, $23.30 under the cheapest Plan G, or $280 a year. What that money buys back: a copay of up to $20 on some office visits, up to $50 on an emergency room visit that does not end in admission, and Part B excess charges, which Plan G pays and Plan N leaves with you. Four office visits in a year is $80 in copays against $280 in premium.

Your County Barely Moves the Floor Here
North Carolina has 100 counties and 73 Medigap rating areas, which sounds like an address problem. It is not. In the September 10, 2026 county pull, the cheapest filed Plan G at 65 was $114.97 in Alamance County and $131.54 in Yancey County, the two ends of the state. The whole geographic range is $16.57 a month.
Set that against $213.36 between two carriers standing in the same city. Moving counties changes your bill by pocket change. Changing companies changes it by $2,560 a year. That is the whole North Carolina cost story in two numbers, and it is why we quote the field rather than a shortlist.
