The cheapest Plan N in Philadelphia County is $114.51, from Insurance Company of North America (Chubb). The cheapest Plan G is $168.59, from Nassau Life Insurance Company. That is $54.08 a month, $649 a year, for a 65-year-old woman who does not use tobacco, from CSG Actuarial filings pulled September 11, 2026.

In most states that saving is weighed against three things Plan N gives up: an office copay, an ER copay, and Part B excess charges. Pennsylvania law bars providers from billing excess charges, so for care inside the state the third item is gone. What is left is copays against $649, and $649 is about 32 office visits.

What Pennsylvania Takes Off the Table

Under federal rules a doctor who does not accept Medicare assignment may bill up to 15% above the approved amount, and that excess is the one Part B cost Plan N leaves with you. Pennsylvania is one of a small group of states, with Ohio, New York, Connecticut, Massachusetts, Minnesota, Rhode Island and Vermont, that forbid the practice for Medicare patients treated in the state. A Pennsylvanian on Plan N who winters in Florida or sees a specialist in Delaware can still meet the charge; one who stays home cannot.

Insurance Company of North America (Chubb) leads Plan N at $114.51 and files Plan G at $173.35, rank 2 of 27. Neither company is first on both, so the letter and the carrier get chosen together. The $54.08 floor-to-floor gap ranks 25 of 28 in our file, on the wide side, which is another way of saying Pennsylvania rewards Plan N more than most states even before its excess-charge law is counted.

Amber gaps are under $20: at United American and WPS Health Solutions the two letters are one office copay apart and Plan G is the only sensible buy. At the other end, 7 carriers price the letters $70 or more apart, led by GPM Health and Life at $133.08; those are mostly companies whose Plan G sits high on the board. AARP / UnitedHealthcare are community-rated, one price at every age.

Philadelphia Is Not the Pennsylvania Floor

9 of the 16 key counties in the file, Lancaster County among them, quote a cheaper pair than Philadelphia County: $133.47 for Plan G and $91.07 for Plan N. The statewide floors are $133.47 for G in Adams County and $91.07 for N in Adams County. The gap between the letters is $42.40 at its narrowest (Lancaster County) and $54.08 at its widest (Chester County).

Both Letters at 65, 70, 75 and 80

24 of 27 Pennsylvania carriers price on attained age, so both letters climb on birthdays. The floor gap goes from $54.08 at 65 to $86.26 at 80, and the median gap from $53.52 to $68.84. The saving from Plan N grows in dollars as you age. So does the number of appointments.
