Indiana's floor, $109.46 from Bankers Fidelity (Atlantic Capital Life) in Allen County, is among the lowest in our 28-state file. In Marion County the same carrier files $118.22, which puts the Part B plus Plan G bill at $321.12 to $536.57 a month (CSG Actuarial filings, pulled September 11, 2026), the $202.90 Part B premium plus Plan G from $118.22 (Bankers Fidelity (Atlantic Capital Life)) to $333.67 (GPM Health and Life).

Rating Method and Who Holds Which Plan

25 of the 27 Marion County carriers price on attained age, so the premium rises on birthdays and the block filing stacks on top. Everence Association is the lone issue-age line at $168.35; its age is fixed at purchase. AARP / UnitedHealthcare is the lone community-rated line at $160.41, with a dearer class at $602.89; age does not enter its price. The highest Plan G filed anywhere in Indiana is $333.67, from GPM Health and Life.

15 of the 27 carriers carry a 2026 effective date on their Marion County rate; the newest is Washington National, September 1, 2026. The NAIC counted 370,851 Indiana Medigap policies as of December 2023: Plan G 202,217 (55%), Plan F 119,959, Plan N 32,798.
