GPM Health and Life charges 3.1 times what Bankers Fidelity (Atlantic Capital Life) does for that same benefit — over 20 years with no further filings, that gap alone adds up to $74,525. It gets stranger: Arizona slices its Medigap map into 14 rating areas across 15 counties, and Bankers Fidelity (Atlantic Capital Life)'s own Maricopa County line, $146.51, runs higher than the $133.19 that same company files in Coconino County. "Humana Insurance Company" sits twice on the Maricopa County board, $199.33 and $354.71 — two different NAIC-coded companies sharing a brand name, not one line that jumped $155.38. The largest is American Benefit Life at +35.0%, effective September 1, 2026, also the newest filing on record. LifeShield National, the second-cheapest Maricopa County Plan G at $155.72 on September 10, 2026, has filed +20.0% for June 1, 2026. Both numbers belong in the same sentence when you compare quotes. A filing is the carrier's change on its Arizona block, not one person's renewal letter, and its effective date can sit months before or after the report that lists it.

The Lettered Plans, and What Each One Leaves With You

No Arizona carrier gets to write its own benefit chart — the lettered plans are federal, identical company to company. On Plan G, the only bill left in your name is the $283 Part B deductible. Plan N shaves the premium in exchange for copays and Part B excess exposure. Plan F stopped taking new buyers on January 1, 2020, the day CMS closed it to anyone newly eligible for Medicare.
