* Files more than one rate class; the table shows the lowest. Top class at 65: Bankers Fidelity (Atlantic Capital Life) $182.62, WMI Mut Ins Co $199.00, AARP / UnitedHealthcare (NAIC 79413) $609.67, Medico Insurance Company $234.42, Washington National $239.91, HealthSpring Insurance (HCSC) $311.27, AARP / UnitedHealthcare (NAIC 84549) $710.50, Blue Cross Blue Shield $365.43. CSG Actuarial filings pulled September 10, 2026, ZIP 85354.
The middle half of the board, 15 carriers, runs from $186.75 (USAA Life) to $243.17 (Mutual of Omaha). That band alone is $56.42 wide, about 4 times the whole county range. At the top, GPM Health and Life's $457.03 is 212% above Bankers Fidelity (Atlantic Capital Life) for identical coverage.
Bankers Fidelity (Atlantic Capital Life) files two classes at 65, $146.51 and up to $182.62. In its upper class it would rank 5th of 29. Which class applies is the carrier's rule, and a quote at your own age and ZIP shows which one you get.
Two names appear twice because two companies file under each: AARP / UnitedHealthcare as NAIC 79413 (community rated, $176.12) and NAIC 84549 (issue age, $231.32); Humana Insurance Company as NAIC 60984 (issue age, $199.33) and NAIC 73288 (issue age, $354.71). Each has its own rate history, so a quote is keyed to the NAIC code, never the brand.

Methodology
- Source: CSG Actuarial licensed API, pulled September 10, 2026. Every number on this page is read from that file when the site builds.
- Profile: female, non-tobacco, individual, no household discount applied. A man, a smoker or a couple pays a different filed rate.
- Geography: each of the 15 counties is quoted through the one ZIP with the largest land overlap; Maricopa County uses ZIP 85354. The file sorts the counties into 14 rating areas.
- Spread: the dearest filed rate minus the cheapest, for one plan letter, one age and one county. The county range is the gap between the cheapest and the priciest county floor.
- Ratio: the Maricopa County carrier spread divided by the county range, set against every state file where that range is above zero.
- Left out: direct-to-consumer and guaranteed-issue classes, bundled Plan G variants, and rows above four times the state median. Classes dropped from this file: Physicians Mutual (Innovative/Household), Universal Fidelity Life Ins Co (Direct to Consumer), Physicians Mutual (Innovative), Blue Cross Blue Shield (BlueValue).
- Filings: CSG's rate-change record on each quote. A percentage is the block's filed change, matched to the carrier by NAIC code.
- Plan F is closed to anyone new to Medicare since January 1, 2020, and this file carries no Plan F rows. Plan G does not pay the $283 Part B deductible.
